Clinical trial management software, or a CTMS, keeps the operational side of a study in one record: sites, enrolment, monitoring visits, deviations, budgets and payments.
Vendors sell it to sponsors, CROs, research sites and academic centres, usually by quote and sometimes by listed plan. This guide ranks twelve products on setup effort, real price, exit terms and vendor independence.
Some vendors pay for visibility. That never touches an entry, the critical parts included, and following a link to a vendor brings us no income. How that works.
In short
What clinical trial management software does
Clinical trial management software coordinates study operations such as sites, milestones, subject visits, monitoring, issues and investigator payments, apart from the data capture and document systems it connects to.
In this order: setup effort, what it really costs, how your data comes back out, whether you can leave, and who each clinical trial management tool is built for. On top of them sits one rule. The top three places are for challengers, which means vendors outside the big names, where the category has any, and the household brands come in from fourth place. For why these five were chosen, and why nothing here gets a numeric score, see how we work.
Subscription CTMS and eTMF with monthly prices listed per plan
Ranked #1 of 12 in Best Clinical Trial Management Software in 2026.
Published pricingNorth America
Every plan lists its study and user limits, and extra users are bought in groups of five. A paid one-month trial costs 49 US dollars and gives Premium Plus features for up to ten users.
The system is multi-tenant, and the vendor says an independent consultancy oversaw its validation. Its EDC link list names more than twenty vendors, and each integrated study needs a monthly support fee plus a one-time setup charge if you want help.
What stands out

Oakland company
Where it costs you
The default hosting location is the United States, and storing data in the EEA is an optional monthly add-on
Validation packages, EDC integration setup and training are priced on top of the subscription
Right for
Small sponsors, CROs and sites wanting a listed monthly price and built-in eTMF
Wrong for
Organisations that need a quote-free route for a large enterprise portfolio
United StatesMonthly plan prices published; annual rate discounted; one-month paid trial; enterprise quoted
Swiss CTMS with built-in eTMF and supply management on SaaS licences
Ranked #2 of 12 in Best Clinical Trial Management Software in 2026.

The vendor says its CTMS runs local, regional and global trials and covers interventional and non-interventional studies. Reports are built from all CTMS data and exported to Word, Excel and PowerPoint.
Standard interfaces exist for Veeva eTMF, EDC systems, project tools and single sign-on. Its technology page says applications ship in containers so a customer can move from one cloud to another, which is the clearest exit statement in this list.
What stands out

Cloud-to-cloud portability
Where it costs you
Electronic data capture comes through partners, so a team that wants EDC and CTMS from one vendor must look elsewhere
No price or trial is published, so budgeting starts with a sales conversation
Right for
European sponsors and CROs wanting one Swiss vendor for CTMS, eTMF and supply
Wrong for
Teams that want to compare monthly prices online before talking to anyone

Site-operations CTMS covering budgets, visits and invoicing for research sites
Ranked #3 of 12 in Best Clinical Trial Management Software in 2026.
Pricing on requestNorth America
The CTMS page promises coordinator time savings from automated scheduling, milestone and visit tracking, and revenue capture from tracked budgets and completed visits.
A bundled offer called SOMS packages the site modules together, and an EDC connection is a separate item in the menu. Check which of the modules you saw in the demo are in the quote, because the vendor sells them as separate solutions.
What stands out
Site and network focus

San Antonio company
Where it costs you
The CTMS description is written for sites, networks and academic centres, so portfolio oversight for a sponsor is not its stated focus
The pricing page asks you to request pricing and lists no amounts
Right for
Research sites and site networks that need budgets, visits and invoicing in one system
Wrong for
Sponsors that want portfolio dashboards across many CROs and countries

Finance is where the page spends its words: visit-level or procedure-level amounts, payables, invoices and payment vouchers, reconciliation of sponsor reimbursements and year-end 1099 reports. Stipends run through Dash Solutions cash cards.
Recruitment includes phone screening questionnaires and booking of prescreen visits. Ask how the US tax reports and cash-card flow apply when your site sits outside the United States, because the page describes them in US terms.
What stands out
Pairs with CRIO eSource
Stipend cards
Regional sign-in incl. EU
Where it costs you
The CTMS is described as designed to work with CRIO eSource, so its value drops if you keep source capture elsewhere
No price or trial is stated on the product page
Right for
Independent sites and site networks that also want electronic source in the same vendor
Wrong for
Sponsors that need a sponsor-side portfolio CTMS across many CRO partners

Ennov says customers can switch between cloud and on-premises at any time and host data locally. Its CTMS dashboards filter by study, country and site and export results, and role-based permissions cover internal staff and outside partners.
IMPACT CTMS adds a mobile monitoring app and investigator payment management. The vendor lists ISO 9001 and ISO 27001 and claims 21 CFR Part 11 support, which you should test in validation.
What stands out

Imports from Ennov EDC
Where it costs you
Ennov sells two separate CTMS products and the page does not explain how to choose between them
The CTMS sits inside a broad Ennov suite, so a buyer wanting only CTMS pays attention to what is bundled
Right for

Salesforce-based CTMS with eTMF, RTSM and portals in one data model
Ranked #6 of 12 in Best Clinical Trial Management Software in 2026.
Pricing on requestNorth America
Because the product lives on Salesforce, sign-in can run through Salesforce accounts, and the site menu mentions open REST APIs for outside systems.
Modules reach beyond CTMS into recruitment, eConsent, study start-up and safety, which suits a buyer consolidating tools but complicates a CTMS-only comparison. Ask which modules are in the quote, how Salesforce licences are counted, and what data export looks like at contract end.
What stands out

Open REST APIs
Where it costs you
The product runs on the Salesforce cloud platform, which ties your exit to that platform
No price or trial appears on the CTMS page, and the module list is long enough to hide extras

The CTMS page frames the product as a single dashboard for milestones and deliverables, plus site and staff credential records and training logs. Financial functions include forecasting, grants and reimbursements across currencies.
The product line also includes an agentic AI package, which the CTMS page does not describe in detail. Request a sandbox with your own protocol and ask who legally contracts with you, since the website does not say.

Reports and letters follow configurable review workflows, and financial records are described as time-stamped and immutable. The page mentions more than a hundred configurable notifications and connectors to third-party EDC, RTSM and eTMF, which matters if you keep an incumbent EDC.
Badges cite 21 CFR Part 11, GDPR and EU Annex 11, but the page does not say how the CTMS meets each. Ask for the validation pack.
What stands out

Where it costs you
Biospecimen management and other extra modules are licensed separately from the enterprise licence
The licence fee depends on protocol volume, so a growing portfolio changes the price

CTMS joined to Rave EDC and eTMF, a Dassault Systemes brand
Ranked #10 of 12 in Best Clinical Trial Management Software in 2026.

The page supports sponsor-led and CRO-managed trials and describes a CRA monitoring workspace with in-context data feeds. Documents uploaded once also update the eTMF, which removes a re-filing step.
Medidata's product pages did not state a headquarters country or hosting arrangement. Ask where data is stored, what the exit export contains, and whether the CTMS can be bought without Rave EDC and eTMF modules.
What stands out

Rave EDC data feeds
Sponsor or CRO managed
Where it costs you
The strongest links are to Rave EDC and Medidata eTMF, so a team on another EDC gets less automation

Dashboards track enrolment and milestones with drill-down, and monitoring visit reports support dynamic question branching. Investigator interactions sync with Veeva CRM for a combined view, and CTMS connects to EDC for enrolment, monitoring and payments.
The page does not state hosting terms or an export route. Ask what a CTMS-only deal costs, because the stated benefits link it to the other Veeva clinical applications.
What stands out

Clinical Transfer for CROs
Where it costs you
The page gives no price and directs buyers to sales
Automatic trip-report filing assumes you also run Veeva eTMF
Right for

CTMS inside Oracle clinical operations modules on Oracle Cloud Infrastructure
Ranked #12 of 12 in Best Clinical Trial Management Software in 2026.
Pricing on requestNorth America
The page says the CTMS connects with EDC and eTMF for data integrity and oversight across a portfolio, and that configurable workflows tie sponsors, CROs and sites together.
Neighbouring modules cover study budgets through ClearTrial, site selection and site activation. Because the product page is marketing copy without feature tables, ask for a configuration walkthrough, a validation approach and the data export format in writing.
What stands out
Oracle Cloud Infrastructure
Planning to CTMS modules
EDC and eTMF interoperability
Where it costs you
The CTMS is one of many clinical operations modules, so the quote scope needs spelling out
No price, trial or export terms are shown on the page
Right for
Large sponsors on Oracle cloud contracts wanting planning plus CTMS
Wrong for
Small research sites looking for a light, quickly adopted tool
United StatesPricing on request; demo and contact form
Clinical trial management software coordinates study operations such as sites, milestones, subject visits, monitoring, issues and investigator payments, apart from the data capture and document systems it connects to. Differences that change the outcome seldom sit in the features. Here is the sequence we would follow.
01
Start with pricing transparency
The split is 1 published against 11 quoted. If you want to compare numbers first, start with SimpleTrials.
02
Ask which country the contract runs through
Jurisdiction is a buying criterion here. The 12 vendors are spread over 4 countries (North America 8, Europe 2, Elsewhere 2), and the home country determines which disclosure rules bind them.
What a CTMS covers and what it leaves to EDC
A clinical trial management system tracks the operations of a study: sites, milestones, monitoring visits, deviations, budgets and payments. Data capture belongs to EDC, and documents to eTMF, but vendors blur the lines. Viedoc and Castor, for instance, sell data capture and electronic patient reported outcomes without naming a CTMS on their home pages, so they are not ranked here.
Ennov CTMS imports study information from Ennov EDC, Medidata CTMS draws from Rave, and Veeva Vault CTMS files trip reports into eTMF. SimpleTrials, by contrast, connects to more than twenty outside EDC systems. Decide first whether you want one vendor for all three layers or a CTMS that accepts your existing EDC, because that choice narrows the list more than any feature table does, and it also decides who you call when a query, a visit report and a filed document disagree with each other.
Write down which EDC and eTMF you already run.
Ask each vendor which integrations are standard and which are billed per study.
Request the list of EDC systems already connected in live studies.
Listed prices versus quotes
Only one product here puts plan prices on the page: SimpleTrials publishes monthly amounts for Sites, Standard, Premium, Premium Plus and eTMF plans, with a 20 percent annual discount, and it also lists add-on fees for EEA hosting, EDC setup and validation packages. Every other vendor asks you to request pricing.
Advarra OnCore says its fee depends on licensed modules and protocol volume, and its biospecimen module is extra. BSI CTMS says upgrades are included in the SaaS fee. Compare like with like by asking each vendor to price the same scenario: number of studies, named users, sites, one EDC link, validation documents and training. A quote without those lines is not a price.
Give every vendor the same study count and user count.
Ask for validation, training and integration charges as separate lines.
Check whether upgrades are included in the subscription.
Who the CTMS is built for: site, sponsor or CRO
CTMS products differ in who sits at the centre. RealTime CTMS and CRIO describe site operations, with budgets, visits, invoices, stipends and recruitment. Advarra OnCore targets academic medical centres, and its sister product Clinical Conductor targets sites and networks. Veeva Vault CTMS and Medidata CTMS describe sponsor and CRO oversight of insourced and outsourced trials.
BSI CTMS names pharma, biotech, diagnostics, med-tech sponsors, CROs and academics. A site-centred tool can feel thin for portfolio reporting, while a sponsor-centred one can lack patient payment and billing flows. Name your role in the first call and ask for a reference customer in the same role and region, because a site tool and a sponsor tool answer different questions even when both are called a CTMS.
State whether you are a site, sponsor, CRO or academic centre.
Ask for a reference customer in the same role and region.
Test one financial workflow, such as an invoice or stipend, end to end.
Hosting, validation and getting data out
Regulated software must be validated, and the hosting region can matter for personal data. SimpleTrials hosts in the US by default and sells EEA hosting for an extra monthly fee. Ennov offers cloud or on-premises deployment and says you can switch at any time. Cloudbyz CTMS runs on the Salesforce platform, which makes Salesforce part of your exit plan. BSI says its containers let a customer move from one cloud to another.
Clinion points to Indian law in its terms while its contact address is in Texas. For every shortlisted vendor, ask for the validation package, the hosting region, the contracting entity, and a sample export of study data in a documented format before you sign, since a contract that is silent on export leaves the question for a day when your leverage is lowest.
Ask which legal entity signs the contract and which law governs it.
Get the validation documents and their price in writing.
Request a sample export of one study before signing.
What goes wrong most often when buying clinical trial management software
Buying a CTMS and an EDC together because the demo flowed well, then discovering the CTMS cannot take your next sponsor's EDC.
Comparing a published SimpleTrials plan price with a quote from another vendor that excludes validation, integration and training.
Choosing a site-focused system such as CRIO or RealTime CTMS for sponsor portfolio reporting, or the reverse.
Skipping the exit question: ask Cloudbyz, Ennov and BSI what data you get back and in which format when the contract ends.
